Beachwood Post-Acute & Rehab

Our father/father-in-law, Ilya V., 92 years old, is in Beachwood rehab after he was discharged from the hospital. There is very friendly and professional staff that provides all required procedures and services. We recommend this facility for post-acute and rehab needs.

I spent a week at Beachwood during Christmas and was very impressed in every way with Beachwood. When my first roommate would not stop talking when I arrived late night, they quickly moved me to a room with a lovely roommate. The staff was always attentive, caring, and knowledgeable. The PT was excellent and even the food was good! I also had a big window with a view, which I realize not all rooms have.

This facility is well managed! All the staff were super welcoming. The place was clean and tidy. The residents looked comfortable and well taken care of. I'd highly recommend sending your loved one here.

This is my candid review. It is difficult for remaining family members to remain strong when they have to witness an ailing parent. My family member was transferred to this facility, and I was grateful for the care he received before his passing. The staff on the 2nd floor treated him with compassion. They even welcomed a faith-based organization to come in and pray with the residents, which meant a lot. The nurses were always helpful and communicative with me. They made a genuine effort to remember me during each visit, which made the experience feel more personal and comforting during a difficult time in my life. I also had the opportunity to speak with Debra and Ella in the business office about the financial aspects of my family member's care and what followed after his passing. They were incredibly supportive and understanding, and guided me through everything with kindness and patience. Overall, I truly appreciate the compassion and respect this facility showed to my family.

As I may have posted over a year ago I have a new update as they have done and made so many changes to Beachwood post acute I can say I am very satisfied with the care my uncle is now receiving. The admin Marilyn has open communication with me. My uncle is extremely happy to be back with the facility. I am content with the care plan and hot dogs for him. His favorite food. I am so grateful and will take this as God's way of saying my uncle will be in good hands again. The care team and the administration is top notch they make us feel like family they have welcomed my uncle with open arms and bolig smiles. I would recommend this facility to anyone

I love medical records. There are so helpful and punctual.

I live in the alley behind this building. The number of times that we hear people crying for help is outrageous. We hear people going on for 10 to 15 minutes crying help. Today I reported them to the California Department of public health. If my loved one was in this facility, I would want somebody to report them for neglect.

Staff is unbelievably inattentive

They could really stand to up their bedsore game. Mom came home with a couple nasty ones. That being said, they did as good a job as anyone can with so many residents. The place is generally busy but mostly clean and the staff, however overwhelmed, is friendly and helpful as possible. It's good if you visit your loved one and stay updated. Bring your loved one home as soon as possible. It really is just a transitional place.

Let me start by saying this facility is, in my opinion, the worst place anyone could ever send a vulnerable loved one. My husband entered this facility in 2023 after it was recommended for their so-called "wound care expertise," and it turned out to be the biggest nightmare we could have imagined. 1. The facility is absolutely filthy. The TV room — where families are supposed to visit — is greasy, dirty, and unsanitary. Staff regularly come in and eat their lunch there, giving families no privacy, and then leave their trash and mess behind. 2. Safety is not a priority. Half of the call lights do NOT work. This was reported to the ombudsman's office multiple times, yet nothing changed. 3. Dietary completely ignores patient health needs. My husband is not supposed to have dairy due to his medical condition, yet he was repeatedly served cheese items, ice cream, and other foods that he absolutely should not have been given. 4. Neglect and mishandling of patients. There were multiple incidents where my husband was brought back from appointments and left unattended. On one occasion, he was literally left to fall onto the floor. In another incident, he fell again and his roommates had to call for help — one of them had to get into his wheelchair and physically go to the nurses' station because no one responded. 5. Abuse and lack of basic human compassion. I once witnessed a patient being given a shower with steam pouring out of the bathroom. The patient was yelling that the water was too hot, and the CNA had AirPods in. Instead of helping, she turned her music up and continued to ignore and abuse the patient. I will never forget that moment. 6. This facility is a nightmare for vulnerable people. Because of the neglect in this place, my husband nearly lost his foot. He developed bedsores so severe they made him septic, left him with permanent deformities, and caused long-term complications that he is still dealing with. Please, do yourself a favor: RUN away from this place. It is unsafe, unclean, unprofessional, and completely unfit to care for human beings. No family should ever have to experience what we went through here.

My uncle entered Beachwood Post Acute for rehab but suffered repeated neglect. Despite multiple calls for help and repeated alerts to staff about serious issues around drains poor meal support, prolonged delays in responding to call lights, and ignoring visible signs of site induced declining health, staff failed to act or acted only after significant delays. From failed promises to dismissed transfer requests, the failure to act in favor of the patient was dismaying and saddening. Please use an abundance of caution employing services of the facility for loved ones as urgent concerns go ignored, and providing the promised care is met with delays or go completely undelivered…. avoid it.

My father was sent to Beachwood Post Acute December, 2024 to regain his strength and walk again after months in the hospital. Instead, neglect there caused a severe fall on **Jan 10, 2025**, fracturing his hip. He had hip surgery the same day and returned to Beachwood. From then on, his care was appalling. Despite being told he had a PleurX catheter, staff failed to drain him regularly, and when they did, amounts weren't documented. On January 16, he had a port placed; by January 18, his WBC was high, but no action was taken until I pushed after he developed a cough. Meals were often left out of reach, leaving him without food or water for hours—sometimes since the day before. Call light responses took hours. He developed painful skin breakdown from poor toileting care. I arrived on Sunday, January 26, to find my father lethargic, not eating or drinking, and gargling as if fluid was in his lungs. I told the nurses he needed to go to UCLA (a block away). They took his vitals and said he was "fine." I refused to accept this and called a family friend who is a physician. She immediately said he needed to be hospitalized. The nurses told me they would get a doctor's order and arrange transportation within two hours. Two hours passed—nothing happened. Four hours later, my father's condition declined rapidly. He couldn't recognize or hear me. Only then did they call 911. At the ER, the doctor told us we were lucky we brought him when we did—he had pneumonia and sepsis. He never recovered and died on February 10, 2025. In summary: Beachwood Post Acute repeatedly failed to provide basic care, ignored clear signs of infection, neglected his nutritional needs, failed to communicate, and delayed life-saving intervention. No family should have to beg for care or fight this hard to keep their loved one alive in a licensed facility. I cannot recommend Beachwood Post Acute to anyone. What happened to my father should never happen to anyone's loved one.

I am a friend or relative of a current/past resident Beachwood Post Acute & Rehab is notoriously bad. Over the years they've had consistently poor reviews. I've had two or three friends there and have had the same kinds of care issues that I think ultimately has to do with understaffing. The nurses, the CNAs, and the folks on the floor are all very pleasant and would like to help, but there's just not enough of them and/or they don't have the proper training. My friend has been at Beachwood and wants to return home. Something that I can say has gone really well is physical therapy. She said that she's doing physical therapy every day. The meals are good. What is not great is the responsiveness of staff in a real specific way of using her call light and not getting a response when she has a need. The last time she was at Beachwood some years ago, her medications weren't either delivered timely or as-needed medications weren't given as needed, and that left her in a significant amount of distress. Her room is more of an institutional setup, like acute care. It always appears clean.

Our friend has been here for 7 months, he has revived excellent care from the staff, including nurses, therapists ( Occupational, Respiratory, and Physical), nurses, CNA's, Admissions and more. Not only are his physical needs met, but the families emotional needs are met by the show of compassion shared with our dear friend.

The staff was attentive and genuinely cared about the patients. Nurses were always available and responsive to any needs, making sure my family member was comfortable and well taken care of. The facility itself was clean and well-maintained, which made a positive impression. They offered a variety of rehabilitation services, including physical and occupational therapy, tailored to my family member's specific needs. The therapists were knowledgeable and encouraging, helping them make significant progress during their stay. I would recommend this center for anyone in need of post-acute care and rehabilitation.

I recently spent some time at Beachwood Post Acute & rehab Center after my surgery, and I can't recommend it enough. From the moment I arrived, the staff was welcoming and attentive, ensuring that I felt comfortable and cared for. The facility is clean and well-organized, which immediately put my mind at ease. The nursing staff was exceptional—always checking in on me and addressing any concerns I had. They made sure I understood my recovery plan. The physical therapy team was fantastic as well, providing personalized exercises that helped me regain my strength gradually. The center also offered a variety of amenities that made my stay more pleasant, such as nutritious meals and comfortable recovery rooms. Overall, my experience at the Center was positive, and I felt well-supported throughout my healing process. I would definitely recommend it to anyone needing post-surgery care!

Beachwood is an excellent place. They are very attentive to their patients. They do many activities with them, like movies and bingo, Etc.
