Pacific Post-Acute

I came to Pacific in very poor health and left in very good health. The people are the reason. The care and concern on everyone's part was outstanding. I truly thought I would go out in a box instead I went out feeling on TOP OF THE WORLD. Thank you everyone!

This Facility is amazing. My aunt stayed here and was treated very well. Awesome staff and very courteous. Nurses were always there checking up on my aunt and making sure she was doing well . Overall I was very pleased and I appreciate the care they provided. Thank you.

Pacific's care and staff are second to none. Everything they do in terms of there diligence and work ethic as well as their caring attitude is all for their patients. Thank you for being so amazing and what you do!

I spent 2 weeks at Pacific to address my disability. What was an important factor to me was the size. 47 patients and only 2 to a room. The crew were wonderful and I received top notch PT and OT. I interacted with a lot of patients during Bingo. It was very refreshing. I even got to watch Monday Night football in the common area. They had many great activities. I even got a great manicure. The most pleasant surprise was the Chef. Retired military and one hell of a creative cook, especially the deserts (even though he is a 49ers fan) vs my Rams. When it was time to leave they helped me pack as I had brought way too much stuff (clothes). A special shout out to Michelle and Mark for taking such good care of me. Sorry it took so long to post but I've been working hard at outpatient PT.

I was just released from Pacific Acute. I have been in one other facility and am familiar with two others. This was the best. The food was the only thing that didn't measure up to the rest of the facility. The nights were quiet which was really important. Staff was adequate for the number of patients. I would definitely choose this facility over others. Wendy, the case manager, actually called me to make sure I was okay two days after I went home.

My husband has been recuperating from two major life-threatening surgeries at Pacific Post Acute for many weeks and he has gotten amazing care! The owner, administrators, nurses, aides, P.T. and O.T. staff, cleaners, food prepares, just everyone has been incredible attentive, compassionate and kind. I see that the staff get along with each other and have made friend with each other. They are happy working there and even travel long distances to work there which tells me they are treated well too. That means a lot to me! I have tears of joy thinking of how well taken care of my husband has been! They even recognized during his stay that something was wrong and perhaps he needed urgent hospital care. They insisted he be taken to the hospital…I believe they saved his life by doing that! Once in the hospital he was rushed to a further emergency surgery. When he was discharged from the hospital Pacific Post had saved his room and bed for him and all his things for him. They welcomed him back so lovingly. The food has also been good and the P.T. wonderful! They give him his meds everyday as directed, on time and correctly. Telling him exactly what it is he is taking. I would highly recommend this facility, though physically not the newest and fanciest, to anyone in need of skilled nursing care. I could not be happier with Pacific Post Acute. Clean professional facility. Personnel across the board were excellent. Michelle Dachot was so helpful in navigating our situation. Nursing and rehab staff are first rate. Salima and Blanca always went the extra mile! I recommend this facility wholeheartedly, Mark and Sammy also very helpful.

Michelle and Mark are both all stars – Michelle is so knowledgeable about this whole complicated process, and Mark went above and beyond to make sure that my dad was getting the personalized care he needs. It's hard having a loved one go to a skilled nursing facility, but the people at pacific post acute are like a family that make the whole process much nicer and more manageable for all.

My father was there for 2 months after refusing to return to different skilled nursing facility following hospital discharge. The staff's attitudes were exceptionally kind and professional. The physical rooms weren't the newest or largest but they were clean and the beds were very good. PT and OT were exceptionally good. The staff definitely went beyond the basics to make sure patients had the best possibility to recover fully. We appreciate the teamwork the most. Everyone tried to help instead of just saying it was someone else's job, they would try to get the right person working your issue. My father reported that the food was very good and generous. Snacks were always available and if we left for medical appts over a meal, they would pack a sack lunch. Exceptionally positive and kind staff. I do suggest that family should always check on arrangements and not assume things happen seamlessly. Thanks to Mark, Diana, Sammy, Liz, Grace, Aimee & Steve plus all the dedicated staff whose names I can't spell.

My dad stayed there. It won't wow you when you walk in the door; I'd give them a 4 for that, but every single staff member I've dealt with there from Sammy (director, I think) or Steve (PT), to the young guy at the front door has been outstanding. The kitchen staff was politely helpful when I brought lunch from home and wanted simple things like a plate and flatware so I could share lunch with my dad. Some people really are on a mission from G-d and getting it done even if there is no band playing when it happens.

My mom recently was at Pacific Post and I am grateful for the excellent care she received. The staff was very supportive and the physical therapy team outstanding, she regained her strength and mobility in a relatively short period of time. I highly recommend Pacific Post to anyone in need of rehab services.

I'm currently a patient here at pacific post acute, was admitted for s/p hip replacement. I've been here for a month, l was hesitant and scared at first, but my family and I know that I needed more help to recover completely. My first impressions of Pacific Post acute was its a very nice looking place, clean and smells good, it doesn't smell like ur typical nursing home. The staff are very good, respectful, caring, professional, clean, compassionate and very hard working. I will recommend this place to any one looking for somewhere to recover and needs rehab. After a few weeks of rehab, with the help of both phenomenal rehab and nursing team, I'm now going home. I can't thank you enough for all the help that all the staff at Pacific Post acute did for me. I want to especially mention the following people that made this experience memorable in a good way for me. Dolores social service, Mark admission coordinator, Michelle marketing, Virgina DON, all my CNA's, Mirla RN, Geraldine RN , Kassandra LVN, Kent DsD, Bianca and Kristy from Rehab and most of all to my favorite Nurse Lili always making me laugh, and always making sure I'm well taken care off. Thank you again team Pacific!!!

Unfortunately, I am not familiar with any of the staff except for receptionist Harley who has such a sweet voice and a helpful demeanor. Currently, my father is undergoing rehabilitative care at Pacific Post-Acute and he seems to appreciate the care he is receiving. Harley acknowledged to me that the phone in the room where my father is was recently replaced and when I spoke with him this morning, Harley was kind enough to increase the volume on the phone since he was unable to hear me. While I am hoping my father won't be at this facility too much longer, I am appreciative of the way he is being treated. When I visited him recently, I was surprised to see he had his hair cut and was freshly shaved! He looked great! I wish I knew the names of the others who are caring for my father; however, since the receptionist is usually the individual who is the "first impressionist", I highly commend Harley on her warm personality, helpful demeanor and lovely voice.

Sending a loved one to a care facility is one of the most emotionally challenging decisions a family can face. For me, placing my father in a facility was the absolute last thing I wanted to do. Too often, care homes are viewed as places where people go to decline—not to heal—and the thought of that weighed heavily on me. When my father was discharged from UCLA Medical Center, he was in rough shape, 94 years old, not able to walk due to a bad urinary infection that he was hospitalized for, I was given a list of recommended facilities. I began researching each one thoroughly, reading Medicare reviews and personally visiting them. The process was emotionally exhausting and, frankly, disheartening. Many of the facilities I visited felt cold and institutional. Staff members seemed unhappy, and the environment felt more like a place to endure than to recover. Then I toured Pacific Post-Acute in Santa Monica, and everything changed. From the moment I walked in, I sensed a different energy. The staff at Pacific Post-Acute didn't just work there—they clearly cared. People were smiling. The team was engaged. I saw patients up and about, socializing and moving—unlike the scenes at other facilities where most patients remained bedridden and withdrawn. There was a palpable sense of dignity and purpose here. I had the pleasure of meeting Mark Tubalinal, who gave me the initial tour. He was not only warm and informative, but incredibly responsive to every follow-up question and request I had, of which there were many. Mark made sure I felt supported every step of the way. His empathy and professionalism made a huge difference in helping me feel confident in the decision. Now, three weeks into my father's stay, I'm beyond relieved and grateful to share that he is thriving. He's walking again, mentally sharp, and gaining strength daily. The transformation is nothing short of remarkable. I owe this to the incredible team at Pacific Post-Acute: Mark and Dolores – Thank you for your constant care and attention, and for treating my father like family. You are always going beyond the call of duty. Christine, Steve, and Jake from Physical Therapy – you have quite literally helped my father get back on his feet. Your skill, encouragement, and commitment have changed his trajectory. Nursing Staff: Without your hard work and dedication, we would not be where we are today, I am grateful for what you do. It's not easy, it takes a very special person. Deron – Thank you for thoughtfully accommodating my father's dietary needs and ensuring his meals are not only nutritious but enjoyable. Each of you plays an essential role, and it shows. You've not only given my father a chance to recover—you've given me peace of mind. Without hesitation, I would recommend Pacific Post-Acute to any family facing the difficult decision of placing a loved one in care. It's a place of healing, dignity, and heart. I had a major back surgery which lasted 10 hours, I stayed in the hospital for 8 days and was released to Pacific Post-Acute for rehab I have had total of 10 surgeries so this isn't my first rodeo. The staff is second to none very attentive and warm the grounds & rooms are very clean and the patients seem to interact very well with one another. The PT & OT team are very knowledgeable about the body and how to rehabilitate it they are warm and demanding in a good positive way. I really believe being a business owner myself that all of the afore mention stems from Great leadership and Guidance start at the top. The person that keep this train running smoothly is Mark!! . Mark over sees everything he is very personal and accommodating with a warm smile and a Whimsical personality. If anyone is looking for a place to rehab look know further ur wasting ur time, I would strongly recommend that you go PICIFIC POST-ACUTE.

I'm currently a resident at Pacific Post Acute. I have fibromyalgia and woke up unable to walk, even one step. One of the administrators, MARK, took on all of my problems and solved them with grace and empathy. MARK's moral compass is always pointed at True North. the staff follows his lead! I feel safe and comfortable here. . you will too. Hope and joy fill the corridors and activity rooms! Be well. Sleep well and sweet dreams.

I highly recommend Pacific Post Acute!! The level of professionalism is top notch and the staff is friendly, experienced and incredibly kind. I feel so fortunate to have found this facility. My mother and I are so grateful for everything they do to make every day as comfortable and supportive as possible.

This facility gave me peace of mind when I visited it looking for a place where my husband could receive specialized rehab nursing care after a short stay in the hospital for mobility issues. The admissions director accommodated my husband's needs and made him feel welcome the first week. The physical therapists work well one on one with my husband and are making progress with him every week. They are caring and understanding of his issues and have tailored a program that suits my husband's needs.

The most important thing to me when a loved one is in need of a post-acute facility is a caring, competent staff. Our experience was that the administration (especially Lynette) was fantastic. The nursing staff was so wonderful (shout out to Erik and Tay, who I had the most interaction with… but the rest of the staff were so wonderful). The therapists are knowledgeable and do a good job. Is it a vacation at the Ritz Carlton? no. Because at the Ritz, they wouldn't provide the difficult kind of care post-acute patients need. No place is "perfect" in a situation like this, because "perfect" would be not having to go to a post-acute. But if you want a place where the staff works incredibly hard and cares, and your loved one is safe and in good hands, Pacific Post Acute is a good choice. Thank you Pacific Post Acute.

Pacific Post Acute's staff is wonderful. In a stressful time for our family, they delivered compassionate, competent care. Our experience was very positive and my mother didn't want to leave when it was time to be discharged. No one likes to be in the difficult situation of having to go to a post acute facility, but Pacific Post Acute was there for us in a big way.

Physical therapy and occupational therapy are very good. They are very trained. They help me a lot. Nurse Alba is very kind and professional and advised me well. I had a good experience here. Lynette is very helpful. I had some trouble with my bed. Parking is on the street. The food was very good, I lost 8 pounds in 1 month.

This facility is spotless, smells fresh, and has excellent staff. I'm a nurse and my beloved grandmother had a hip injury and ended up here for rehab. The staff really care about their patients. They'll get them up every single day and dress them for success. It's a privilege to go visit my grandma because I know she's in good hands and she loves it here. Not to mention they specialize in dementia patients. I interact with Lynette, Debbie, and Elizabeth they love what they do and it shows. I'm thankful for their kindness and top-notch level of care. And Jarron is the chef of the year. He cooks with love and knows all the patients by name and their special preferences for food. Thank you all!

The Facility and especially the Staff are excellent. This is the first time any close family member of mine has needed a Skilled Nursing so I was uncomfortable and unsure of what the place was going to be like for my Father, 81 years old. From the Nurses, Assistants, PT, OT, Speech Therapists, Social Services, and everyone I forget to name, they were all so caring and truly worked hard to encourage him to take his meds regularly, go to therapy, and eat good hearty healthy food. In 2 weeks he gained a needed 10 lbs and learned how to use a cane. Many thanks to everyone. This was the best choice for my family and I hope it will be for yours too.

Nurses are very attentive to Residents they are prompt, polite and courteous. Social activities encouraging community during the pandemic have been very helpful along with FaceTime, WhatsApp and Zoom video visits with family members. In short this is a great and caring environment for your short term and long term care needs for your loved one.

My grandmother has been to multiple facilities which included both great and difficult experiences. We were referred to this location by a family friend, and have been pleased with the professionalism and care that has been provided to our family member. My grandmother has been happy with the staff which in return validates that we made the right decision choosing this location. We feel at peace knowing that she is getting the proper attention and care that she requires.

My sister in law discharged from Pacific Post Acute earlier today after being there for about 3 weeks for rehab. She was a bit scared to leave because of what a great job they did. The therapy and nursing team worked miracles with her and we all feel like she's now ready to come home. They gave her the tools to be successful and even helped train us a bit to make sure we can help. The staff was always attentive to her needs, even while busy. She was there during a transition with administrative staff, and she fell in love with the new guy because of how friendly he was in making sure she was ok and happy.

I cannot tell you how pleased I was with the facility, administrators, Drs, P.T. Dept. Staff, Adjunct Staff, Nurses, CNAS housekeeping and custodial staff. Pacific is a small facility. It is homey and many languages are spoken. The atmosphere is warm and the staff inviting and always helpful. Pacific is by far the best Care Facility in which I have chosen through word of mouth to recover. From Stephanie in Admissions to Rachne who will fulfill any special need you may have. Kel, Kevin, Edwin, Kyle LVNs Jonathan,, Francesca, Amber, Nora, CNAs, Tom, Carmen, Liz, Liza, Daniel, Lin, Lisa, PTs., Carlos, Maintenance and Lupita, Housekeeping you will be served with utmost attention and care.

Everyone here is super nice. My mom's health is much better since she started her stay here and she is really happy with the loving care she receives here. I highly recommend this place. Thank you Daniel, Hailey, James, Tanya and everyone else here.

I was released two days ago from Pacific Convalescent Hospital following surgery at UCLA Santa Monica. I will admit I was in a great deal of pain and all I wanted to do was go home but I was not ready to go home alone. But the care I received there prepared me to go home and I am doing very well!! Doctor Garcia and her team of nurses, healthcare providers and physical therapy team put me back on my feet. Once I was permitted real food following a clear liquid diet and then a pureed diet, the food was really very good! I am sure, if the necessity ever arises again, I would return to Pacific Convalescent Hospital without a moments hesitation. They are a great team of healthcare professionals, and I sincerely thank them for the weeks of care they provided me!

What an awesome group of health care professionals! The CNA's,like Gloria and Abena, the physical therapists,occupational therapists, speech therapists, like Daniel, Tom, Elizabeth, the director James, the nurses like Kevin, Salima and Garett, the activities coordinator,Esperanza, the accountant,MaLou, the receptionist, Stephanie,the doctor assigned to the facility Dr. Garcia, were all caring, helpful individuals. Everyone there went out of their way to make my mother feel comfortable. They all watched out for her, treated her with kindness, love and respect.

I have seen and visited other facilities in the past, and am thankful we were able to have both my mom and grandma at Pacific Post Acute. Having a loved one in a facility is never ideal, and while seeing several sick and frail patients can be jarring, I feel as thought Pacific Post Acute does the best in comparison to all facilities I've seen before. The fact that they are small is also a huge plus. Most of the issues we raised were addressed and resolved in a timely manner. The majority of the staff was great with my mom and grandma. Some of the nurses and CNAs became like family after several months of having my mom and grandma placed there. Kevin and Garret were the best nurses, and the CNAs (Salima stands out the most, with her goofy jokes and caring ways) were always concerned with their well-being. It's not an easy job, but you can tell which folks are there because they care about the patients. I also loved that they had several people arranging activities for the residents. Activities I also participated while visiting, and felt they were entertaining and not as forced as other places. Additionally, the patio area was a great amenity. It was nice to be able to roll them out to a tranquil space with fresh air.
