10 YEARS LATER, I AM THANKING YOU WITH A YELP! When I was choosing a SNF for my aged Grandmother, I visited a dozen or more places in the Ventura/LA County area. I believe my grandmother was there for nearly 7 years. As she aged and her health declined, slowly at first...and then quickly worse in the last few months of her life, the entire staff was lovely and supportive at each stage. While anyone looking needs to do their own review and choice, I can't say enough about my experience as a caregiver...just how wonderful the care was for my Grandmother, how clean the facility is kept, and how patient and loving the support staff was to me. They earned my trust and were there in difficult times. Give them a chance to host your loved one, review their facility, meet a nurse or two, and do your own evaluation. I recommend them highly, and thank them so much!

After visiting 3 facilities to bring my mom to after a short hospital stay from a fall, I chose Thousand Oaks Post Acute and I am so glad that I did. I chose it because of the warm and inviting vibe I felt when I walked in. It was just that for the 20 days that my mom was there. We had a great experience. The room was comfortable with a nice big window. The food was not so bad either. The nursing staff was diligent and very friendly. They made my mom feel loved. The Physical Therapy department and therapists were amazing! They worked with my mom everyday and gave her something to look forward to. Apparently, my mom "rocked" at pt. The administration staff was also very helpful and answered all my questions. The process played out just like they told me it would when we first came. I would not hesitate to use Thousand Oaks Post Acute Center again if needed.
My 85-year old father was living with me since my mother passed away 11 months ago. His mobility, cognition and personal skills were on the decline and a month ago, he had an episode which required a 6 day hospitalization. He was discharged to skilled nursing and I selected Thousand Oaks Post Acute. I could not have been more impressed with the staff and services that were provided for my father during his brief 20 day stay at the facility. He received wonderful physical and occupational therapy. The food was nutritious and tasty. Most of all, everyone from the nurses aides to the RNs and administrative staff were top notch. They treated my father with respect and warmth. They genuinely cared. At the end of his stay, my dad developed pneumonia and was placed briefly on hospice care. Vita's Hospice worked seamlessly with the TO staff and my father received tremendous attention and care until he passed peacefully in his sleep. If anyone is searching for skilled nursing for a family member or friend, I highly recommend Thousand Oaks Post Acute. Thank you to Glenda, Jennifer, Neil, Sheila, Jason, Denise, Christina and Donna.
